Banned permanently for taking:
1) two same accounts, without reporting one as dead
2) first one was and still is working - have checked it
3) then Rules-disobeyment got him and he took another one and still didn't report it as dead
I would have forgive you, but the first one (IS STILL WORKING) and you needed another spare one.
